Supporting Patients and Families at End of Life

Caring for patients and families near the end of life is complex and deeply personal. Transitions LifeCare’s educational offerings address the many dimensions of care, including emotional, physical, psychosocial, and spiritual support.

These classes can help healthcare teams feel more confident when having difficult conversations, recognizing changes near end of life, supporting grieving families, and managing common symptoms such as pain, shortness of breath, anxiety, agitation, nausea, and restlessness.
