Transitions Complex Illness Care provides an added layer of support for patients and families navigating advanced, chronic, or life-limiting illness. Our team works alongside your current doctors and specialists to help manage symptoms, clarify goals of care, coordinate resources, and support quality of life at any stage of illness.
Care may be provided at home, in assisted living or skilled nursing facilities, or in hospital settings, depending on each patient’s needs. Transitions LifeCare’s palliative care page notes that its consultative services support symptom management, goals of care, end-of-life planning, and access to community resources while collaborating with the patient’s primary medical team.
Complex Illness Care is designed for patients who need additional support while continuing to see their primary care provider, specialists, or other members of their healthcare team. Patients may continue treatment and other services while receiving care from Transitions LifeCare.

The team can help with symptoms related to serious illness, including pain, shortness of breath, anxiety, depression, appetite loss, and other concerns.
