Transitions Kids supports children and families facing serious illness, from perinatal care through age 18. The program focuses on quality of life, symptom support, care coordination, and compassionate guidance for the entire family.
Pediatric Palliative Care
Pediatric palliative care provides an extra layer of support for children with serious illness and their families. It can be provided alongside ongoing treatment and works to improve comfort, manage symptoms, and help families navigate complex care needs.
Palliative care may include:

Pediatric Hospice Care
Pediatric hospice care offers more intensive support when a child is nearing the end of life. The focus is on comfort, dignity, and quality of life, while surrounding the child and family with compassionate care.

- Coordination with existing providers
- Ongoing support for the family
Care That Works Alongside Your Child’s Providers
Transitions Kids works alongside your child’s doctors, specialists, hospital team, and other providers. The goal is to make care feel more coordinated, supportive, and centered on what matters most to your family.
Where Care Is Provided
Most children receive care at home, but support may also continue in the hospital. In some situations, hospice home care may also be available when needed.
24/7 Support for Families
Families have access to support around the clock. A nurse is available by phone or video 24/7, and children receiving hospice services may also have access to in-person nursing support at any time.
Bereavement Support
Support does not end after a loss. Transitions Kids also provides bereavement care for families, helping parents, siblings, and loved ones navigate grief with ongoing support.
